How does Palliative Care differ from
Hospice Care?
• The patient has a prognosis of 12 months or less of life
expectancy.
• Emphasis is on improved pain and symptom management.
• The patient may continue curative treatment.
Nehamah Palliative Care

• An interdisciplinary team, consisting of a physician, nurse and
social worker, whose focus is on the patient and family.
• Enhanced comfort through comprehensive care management, which
focuses on each patient’s physical, emotional and spiritual needs.
• Home visits to provide medical care and education as needed.
• Empowerment of patients and their loved ones to make informed
decisions about end-of-life care.
